The thyroid gland is a small, butterfly-shaped structure that sits at the base of the throat and regulates virtually every metabolic process in the body. For a significant percentage of midlife women, it is quietly malfunctioning.
Autoimmune thyroid disease affects women at a rate roughly seven to ten times higher than men. The symptoms overlap so completely with perimenopause, depression, and 'just getting older' that they are frequently missed, misattributed, and undertreated.
The full picture of thyroid function requires measuring not just TSH but free T4, free T3, and thyroid antibodies. These are standard laboratory assays, but they are not routinely ordered.
The woman who has been told that her thyroid is 'fine' and continues to feel unwell deserves a more thorough conversation — about what 'fine' means, and for whom.
Nutritional factors matter as well. Selenium is essential for the conversion of T4 to active T3. Iodine is necessary for thyroid hormone synthesis but excess can worsen Hashimoto's. These are reasons to pay attention.
